Occupational Therapist (Home Health)

Freedom at Home is a Veteran-Owned Home Health Agency serving Southeast Michigan. We're looking for a compassionate, motivated Occupational Therapist to join our growing team.

Responsibilities
  • Evaluate patients in their homes and develop individualized treatment plans.

  • Provide skilled occupational therapy services to improve independence and quality of life.

  • Assess home safety and recommend adaptive equipment or modifications.

  • Educate patients and caregivers on therapeutic exercises, self-care techniques, and safe use of assistive devices.

  • Complete OASIS assessments and clinical documentation accurately and on time.

  • Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.

  • Monitor patient progress and adjust treatment plans as appropriate.

  • Promote safe, compassionate, high-quality patient care.

Qualifications
  • Graduate of an accredited Occupational Therapy program.

  • Current Michigan Occupational Therapist license.

  • Current CPR certification.

  • Minimum of 2 years of Occupational Therapy experience preferred.

  • Home Health experience is a plus but not required.

  • Valid driver's license, reliable transportation, and auto insurance.

PHYSICAL REQUIREMENTS

On a daily basis: walking, standing, sitting, reaching, use of telephone, use of computers & other office equipment, lifting/transferring 150 lbs or greater, bending, stooping, climbing stairs.
