Job description

Position: Home Dialysis Registered Nurse (RN)
Location: Bassett Health Network (Cooperstown, Oneonta, and Little Falls, NY)
Schedule: 4 x 10-hour shifts, 6:00 AM - 4:30 PM
On-Call Requirements
• Weekday call coverage: 4:30 PM - 6:00 AM
• Weekend call coverage: 24-hour coverage Saturday and Sunday
• Call rotation is typically every other week
Patient Population
• Approximately 14 home dialysis patients across the network
• Primary patient population is Peritoneal Dialysis (PD)
• Program supports both:
o Peritoneal Dialysis (PD)
o Home Hemodialysis (HHD)
Responsibilities
• Manage outpatient home dialysis clinics
• Conduct patient assessments and ongoing clinical management
• Provide modality education and training for new home dialysis patients
• Troubleshoot and triage patient concerns and treatment-related issues
• Coordinate care and support patient independence in the home setting
• Occasionally assist inpatient nursing units with CAPD exchanges
• Complete initial home visits for new patients as needed
Travel Requirements
• Travel between clinic locations in Cooperstown, Oneonta, and Little Falls may be
required
• RNs are only expected to travel to patient homes for the initial home
assessment/visit
• Ongoing patient care is primarily provided in the clinic setting
• Personal vehicle required
Orientation
• Agency nurses typically shadow an experienced team member during their first
week
• After orientation, nurses are expected to independently manage their assigned
patient population and responsibilities
Preferred Experience
• Previous Home Dialysis experience is strongly preferred
• Peritoneal Dialysis (PD) experience is highly desirable, as the majority of the patient
population utilizes PD therapy
• Experience educating and training dialysis patients is preferred
• Strong assessment, triage, and patient education skills are essential
Ideal Candidate
• Independent and self-motivated clinician
• Comfortable managing a small patient panel across multiple clinic locations
• Experienced in troubleshooting dialysis-related issues and providing patient
education
• Adaptable and able to work autonomously after a brief orientation period
• Comfortable participating in a rotating on-call schedule
