Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated:

Registered Nurse (RN) Home Health 

Shift Available: Full Time

Service Areas with Openings: Northeast, Northwest

Responsibilities:

  • Oversee nursing care team.
  • Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care. 
  • Plan, organize, and direct home care services.
  • Conduct assessments of patient care needs and develop care plans.
  • Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes. 
  • Prepare timely, complete, and accurate clinical notes.

Qualifications:

  • Valid RN license in the State of Texas or compact license.
  • Bilingual (Fluent in Spanish) encouraged to apply
  • Home health experience (at least 1 year).
  • Knowledge of home health regulations and general healthcare compliance.
  • Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
  • Excellent communication and people skills are essential.
  • Ability to work independently and as part of a team.
