Job description

Registered Nurse – Hospice

Schedule varies. Typical in-home RN schedule is Monday to Friday, 8 AM to 5 PM with on-call rotation.

Contract Length: 13 weeks Guaranteed Hours: 40

Requirements:

  • Current RN license in the state
  • Valid driver's license and reliable transportation
  • 2 years of recent acute care, home care, or hospice experience preferred

Roles & Responsibilities:

  • Provide skilled nursing and coordinate care for patients and families in a residential setting.
  • Observe, assess, evaluate, and plan interventions based on patient's physical and psychosocial status.
  • Oversee care provided by Home Health/Hospice Aides and CNAs and evaluate compliance with plans of care.
  • Ensure continuity of care through effective communication and coordination with other disciplines and agencies.
  • Maintain accurate and complete patient records per agency standards.
  • Participate in quality improvement activities and in-service training.
