Monday - Friday 8a - 5p
Pay Rate: $70/h
Job Summary:
The Home Care RN (CHHA) provides high quality nursing care to clients in their homes. This role involves assessing medical needs, implementing individualized care plans, working as part of an interdisciplinary team, and ensuring compliance with state and federal regulations. The RN CHHA supports patients across Medicaid, Medicare, and private pay programs, with the goal of helping them live safely and independently at home.
Key Responsibilities:
  • Conduct comprehensive nursing assessments in clients'homes, including physical, psychosocial, and environmental factors.
  • Develop, implement, and evaluate individualized care plans in collaboration with physicians, family, caregivers, and other care team members.
  • Provide direct nursing care: administer medications, wound care, injections, IV care if applicable, monitoring vital signs, and other treatments as ordered.
  • Provide home based patient advocacy;assess barriers and support social, environmental, and emotional needs.

Qualifications:
  • Valid Registered Nurse (RN) license in the State of New York.
  • Experience working in a home health setting or CHHA.
  • Familiarity with Medicaid, Medicare and/or managed care models.
  • Strong assessment, clinical judgment, and care planning skills.
  • Ability to work independently in client homes with minimal direct supervision.
  • Excellent communication skills: verbal, written, and with diverse populations.
  • Proficiency in EHR systems and ability to maintain accurate documentation.
  • Ability to travel to patient homes (valid driver's license / access to reliable transportation as required).
  • CPR/BLS certification required.

Working Conditions & Physical Requirements:
  • Home visits across various client homes;may involve climbing stairs, lifting or transferring patients, bending, kneeling, etc.
  • Flexible schedule, may include evenings, weekends, or on call duties depending on client need.
  • Exposure to infectious diseases and bodily fluids;must use PPE and follow safety protocols.
