Home Health Occupational Therapist

You'll empower independence and restore daily function for individuals recovering from illness, injury, or hospitalization. Key Responsibilities

  • Perform in-home assessments to determine functional impairments and therapy needs
  • Develop and implement individualized plans of care that support safety, self-care, and mobility
  • Provide hands-on interventions focused on ADLs, home safety, and patient-specific goals
  • Educate patients and caregivers in the use of adaptive equipment and techniques
  • Collaborate with physical therapists, nurses, aides, and physicians for coordinated care
