This remote position offers the flexibility to work from your preferred location while providing essential services to clients at our hospice center in Peoria, Arizona. • Conduct thorough psychosocial assessments to understand clients' mental health concerns, strengths, and available resources.
• Provide individual, family, and group therapy using evidence-based treatment modalities tailored to client needs.
• Implement techniques from cognitive-behavioral therapy, solution-focused therapy, or trauma-focused therapy to aid in clients' recovery.
• Develop and enforce individualized treatment plans, which include setting specific, measurable goals and suitable interventions.
• Participate in collaborative discussions with psychiatrists, psychologists, and other healthcare professionals to ensure comprehensive and coordinated care.
• Offer crisis intervention services, performing risk assessments and safety planning to ensure client safety and well-being.
• Maintain meticulous and timely documentation that complies with ethical and legal standards, ensuring the integrity of client records.
• Lead group therapy sessions, psychoeducation classes, and facilitate support groups to foster community and healing.

In this position, you will primarily work with clients facing significant emotional and psychological challenges related to terminal illnesses while also providing support to their families. Your role will involve not just therapeutic interventions but also facilitating end-of-life discussions, bereavement counseling, and advocating for the emotional needs of both patients and their loved ones.

• A Master’s degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW)
• A minimum of two years of experience working in a mental health or outpatient environment, showcasing your ability to manage diverse cases.
• Deep understanding of evidence-based therapeutic modalities and a collaborative mindset to create personalized care plans.
• Proficient interpersonal, communication, and assessment skills to effectively interact with clients and the multidisciplinary team.

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with virtual therapy tools and platforms for delivering care if applicable during remote sessions.
• Knowledge of community resources and referral pathways relevant to the client population you serve.
