Associate Director, Health Services at JASA

As the Associate Director of Health Services, you will provide clinical and operational leadership across multiple core teams: Care Transitions, Medication Adherence, Social Determinants of Health (SDOH), Palliative Care, and Health Home. This dynamic leadership role combines direct staff supervision and clinical training with strategic partner management, program evaluation, and process improvement to expand JASAโ€™s health services footprint across NYC.

Why Youโ€™ll Love Working at JASA

JASA is one of NYCโ€™s largest and most trusted nonprofits serving older adults. Our dedicated professionals help improve the lives of thousands of seniors through impactful, mission-driven work.

Strategic Healthcare Leadership: Oversee high-impact health programs in partnership with major healthcare systems, health plans, and hospitals.

Multidisciplinary Scope: Guide diverse teams delivering Care Transitions, Palliative Care (New York Center for Dignity and Support), and Health Home services.

Innovation-Driven Culture: Opportunity to apply process improvement, workflow automation, and data analysis to shape modern healthcare delivery.

What Youโ€™ll Be Doing

You will balance staff supervision, training development, data analysis, and relationship management to ensure high-quality, sustainable service delivery.

Key Responsibilities:

Team Supervision & Training: Provide direct supervision for Care Transitions Specialists (CTSs); recruit, train, and develop clinical refreshers to ensure high-quality client care. Serve as a substitute CTS when needed.

Medication Adherence Oversight: Act as program liaison and trainer for medication adherence initiativesโ€”mitigating barriers, supporting 90-day conversions, and assisting high-risk non-adherent members.

Palliative Care Management: Partner with the Palliative Care Supportive Services team to oversee the New York Center for Dignity and Support, ensuring program quality and monthly target achievement.

Health Home Integration: Oversee the Health Home program, drive program growth, and foster integration between Health Home and Care Transitions workflows.

Partner & Business Development: Strengthen relationships with hospital systems and health plans; lead partner meetings, prepare case studies/presentations, and support business development efforts.

Program Evaluation & Reporting: Manage program evaluation efforts, complete aggregate monthly reports, and use data analysis to optimize program strategy and operations.

What Youโ€™ll Need

Education: International Medical Graduate (IMG) or equivalent degree required.

Language Skills: Bilingual English/Spanish required (ability to read, write, speak, and understand both languages).

Leadership Experience: Minimum of 3 years of managerial and leadership experience with a proven track record of staff training and skill transfer.

Technical & Process Skills: Advanced proficiency in Microsoft Office and Google Workspace; experience with Electronic Health Records (EHRs). Experience with process improvement, workflow automation, Python, AI tools, or data analysis techniques is highly desirable.

Sector Expertise: Background in hospital discharge planning or healthcare settings preferred.

Travel & Physical Requirements: Ability to travel to clients' homes via mass transit and negotiate steps/physical barriers in residences.

Core Values: Professional maturity, innovative problem-solving skills, and a deep appreciation for how race, religion, ethnicity, and sexual orientation impact client care.

Benefits at a Glance
  • Health & Wellness: Medical, dental, and vision coverage, plus Employee Wellness Programs and an EAP.
  • Retirement Savings: 403(b) plan with Roth option and an employer-funded pension.
  • Generous Time Off: Four weeks of vacation , plus holidays, floating holidays, and sick days.
  • Professional Growth: Senior leadership development, innovation opportunities, and specialized clinical trainings.
