Position SummaryThe Registered Nurse is responsible for providing skilled nursing care to hospice patients in their homes or place of residence. This role coordinates patient care with the interdisciplinary team, physicians, families, and referral sources to ensure quality, compassionate care throughout the hospice journey. Essential ResponsibilitiesPerform comprehensive patient assessments to identify physical, emotional, psychosocial, and environmental needsDevelop, implement, and update individualized plans of careProvide direct nursing care and symptom managementCoordinate services with physicians, aides, LVNs, social workers, chaplains, and other team membersEducate patients and families regarding care needs and disease progressionEnsure accurate and timely clinical documentationMaintain compliance with hospice regulations and payer requirementsParticipate in on-call rotation as assignedPromote HOST values, standards, and quality patient careOther duties as assigned QualificationsEducation:Graduate of an accredited Diploma, Associate, or Bachelor School of NursingLicensure:Current Texas RN licenseCurrent Texas Driver's LicenseExperience:Minimum two years RN experience in a clinical setting preferredHospice experience strongly preferred Skills:Strong clinical nursing judgmentExcellent communication and interpersonal skillsCompassionate bedside mannerAbility to work independently and collaboratively Requirements:Reliable transportationCurrent auto liability insuranceAbility to travel locally throughout service areaCPR certificationAbility to lift up to 50 lbs. and assist with patient mobilityAbility to work flexible schedule and occasional on-call needs Why Hospice of South Texas?Hospice of South Texas is proud to be a nonprofit leader in compassionate end-of-life care.
