Job description

General Position Summary: The Lead Nurse leads the assigned area in order to provide professional, team-based medical care and nursing support in a clinical setting. Under direct supervision of the Chief Nursing Officer, the nursing staff plays a critical role in the delivery of quality, patient centered medical care within the context of a federally (HRSA) qualified community health center (FQHC). The Lead Nurse oversees the assigned nursing team with the goal of providing optimal patient care. The Lead Nurse is a mentor, resource, and example to all staff in the assigned work area.

Essential Duties and Responsibilities:

Patient Relations:

1. Communicates with patients in a courteous and professional manner in face-to-face encounters and via the telephone.

2. Educates the patient and family members on the patient's diagnoses and health care self-management within the scope of knowledge and authority.

Staff Relations/Team Building:

1. Participates in the implementation of staff development and education.

Team-based Care Model:

1. Leads daily huddles and participates in monthly care team meetings to communicate and collaborate on patient issues and needs.

Clinical Duties:

1. Conducts patient screening for hearing, vision, vital signs, blood pressure, and other routine procedures.

2. Assists the providers with in-office tests and procedures as ordered.

3. Administers medications, including intramuscular (IM) and subcutaneous injections as ordered by the provider.

4. Initiates and monitors the infusion of IV fluids if needed, provided the Lead Nurse is a Registered Nurse.

5. Documents all clinical activities and patient interactions in the electronic health record (EHR).

6. Assists providers and the referral staff with referrals if needed (scheduling, tracking, obtaining reports via phone/fax).

7. Collects specimens from patients, performs in-house laboratory testing, and processes specimens and requisitions for transport to the outside testing facility.

8. Stocks exam rooms and special treatment rooms with necessary supplies and indicates the necessity for ordering supplies on the appropriate supply order form, designated board, or list.

9. Demonstrates enthusiasm and commitment toward continuous learning and developmental opportunities.

10. Returns patient phone calls promptly.

11. Demonstrates flexibility, open-mindedness, and versatility in adjusting to changing environments and requirements as necessary.

12. Functions as a triage nurse.

13. Interacts with other agencies/medical entities regarding patient medical information and documents referrals as necessary.

14. Participates in the education of nursing and medical assistant students as assigned.

15. Physical attendance is an essential element of the job and necessary to perform the essential functions of the Lead Nurse position.

16. Coordinates vaccines for VVFC and Private Stock. Obtains and logs refrigerator/freezer temperatures two times a day or designates another staff person to do so. Orders vaccines.

17. Performs other clinical duties and responsibilities as assigned by the CNO.

Supervisory Duties:

1. Identifies attendance, performance, and behavioral issues of team members, consults with Human Resources and/or CNO, and addresses with staff member.

2. Participates in the interviewing process for new team members.

3. Prepares and conducts 90-day (new hire) and annual Employee Performance Evaluations (EPE) for team members.

4. Creates and updates working schedules for assigned location.

5. Assists the CNO in scheduling medical assistant and nursing student rotations.

6. Edits and completes all assigned staff's timecards and time-off requests.

7. Oversees staff orientations with newly hired or transferred staff members to the team.

8. Serves as a resource for all internal staff and external customers.

9. Assists the CNO with creating, reviewing, and editing policies and procedures and is a back-up for the CNO when the CNO and Associate Director of Nursing (ADON) are both absent.

10. Educates and leads the department in all emergency situations and is part of JHC's Emergency Preparedness team.

Performance Improvement Activities:

1. Attends all nursing staff/department meetings and participates in the JHC CQIP program.

Safety/Infection Control Activities:

1. Attends all mandatory OSHA infection control trainings and follows general OSHA guidelines for infection control, including "Standard Precautions".

2. Abides by all JHC infection control policies, including acute response measures in the setting of a public health concern or disease outbreak.

Competencies:

1. Performs annual skills evaluations and signs the RN/LPN/Medical Assistant Nursing Competency Skills Form for the assigned staff.

Other Functions:

1. Staff members will abide by the Code of Conduct as documented in the Corporate Compliance Manual.

Physical Demand and Working Environment: Fast-paced office setting with travel to other offices often. Lifting and/or exerting force up to 25 pounds occasionally, with frequently moving of objects. Work requires speaking, sitting, bending, walking, standing, hearing, and stooping, kneeling, and repetitive motion with certain activities. 8 hours of constant computer usage. OSHA high-risk position.

Qualifications:

1. Must be licensed with Virginia State Board of Nursing with a current RN or LPN license.

2. Must have graduated from an accredited nursing program.

3. Minimum of 3-5 years' nursing experience; prior supervisory experience preferred.

4. Ability to work a flexible schedule.
