Position Summary
Join our in-house therapy team, where you'll help residents regain strength, improve mobility, and achieve greater independence through individualized rehabilitation. Working under the direction of a licensed Physical Therapist, the Physical Therapist Assistant (PTA) provides evidence-based treatment while building meaningful relationships with residents across Skilled Nursing, Assisted Living, and Long-Term Care. Key Responsibilities
  • Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
  • Implement individualized treatment plans designed to improve mobility, strength, balance, endurance, pain management, and functional independence.
  • Observe resident progress, evaluate treatment responses, and communicate findings to the supervising Physical Therapist.
  • Assist with resident assessments, goal development, and discharge planning as directed by the Physical Therapist.
  • Educate residents and families regarding therapeutic exercises, mobility techniques, safety, and home exercise programs.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, and the interdisciplinary care team to coordinate resident care.
  • Ensure therapy equipment is maintained in a safe, clean, and functional condition.
  • Participate in quality improvement initiatives, infection prevention activities, continuing education, and departmental meetings.
  • Promote a compassionate, resident-centered environment focused on restoring function, maximizing independence, and improving quality of life.

Requirements
Required Qualifications
  • Associate degree from an accredited Physical Therapist Assistant program.
  • Current Physical Therapist Assistant (PTA) license in the State of Kentucky.
  • Current CPR certification (or ability to obtain within required timeframe).
  • Strong knowledge of physical therapy treatment techniques and rehabilitation principles.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.
  • Commitment to compassionate, resident-centered care.
  • Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, transferring residents, and assisting with therapeutic activities.
Preferred Experience
  • Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute care.
  • Experience working with geriatric residents and medically complex patient populations.
  • Knowledge of Medicare Part A, Part B, PDPM, rehabilitation documentation, and electronic medical records (EMR).
  • Experience providing evidence-based treatment for orthopedic, neurological, cardiac, and balance disorders.
  • Strong clinical observation, time management, and problem-solving skills.
  • Passion for helping residents achieve their highest level of function and independence.
