Location: Michigan – Covering 2 Counties

Setting: Assisted Living & Memory Care Facilities

Employment Type: Full-Time

M-F-Day Shift


Position Overview


We are seeking a Behavioral Health Registered Nurse (RN) to provide compassionate, patient-centered care to individuals residing in assisted living and memory care facilities across two counties. This role focuses on behavioral health assessment, care coordination, patient education, and collaboration with facility staff and healthcare providers.


Key Responsibilities

  • Conduct comprehensive behavioral health assessments and nursing evaluations.
  • Provide skilled nursing care to patients in assisted living and memory care settings.
  • Monitor and manage patients with behavioral health and psychiatric conditions.
  • Identify changes in mood, behavior, cognition, and overall health status.
  • Develop and implement individualized plans of care.
  • Provide patient and caregiver education regarding medications, disease management, and safety.
  • Collaborate with physicians, advanced practice providers, facility staff, and interdisciplinary teams.
  • Coordinate care and communicate changes in patient condition appropriately.
  • Complete accurate and timely clinical documentation in accordance with home health and regulatory requirements.
  • Travel throughout two assigned Michigan counties to see patients.


Qualifications

  • Active, unrestricted Michigan RN license required.
  • Previous behavioral health/psychiatric nursing experience required.
  • Home health experience strongly preferred but not required.
  • Experience working with older adults, dementia, Alzheimer's disease, or memory care patients is highly preferred.
  • Strong assessment, communication, organization, and clinical decision-making skills.
  • Ability to work independently and manage a geographically assigned patient caseload.
  • Valid driver's license and reliable transportation required.
