Job description

Nurse Practitioner / Physician Assistant — Assisted Living Facilities

Western Geriatrics and Neurology | Part of Brigade Health

Coverage area of Central and North Orange County such as Orange, Anaheim, Whittier, Fullerton, Yorba Linda, Buena Park, Brea and surrounding cities.

We're looking for an NP or PA to round on residents at assisted living and board and care facilities in the area, with openness to providers who'd like to also see patients in skilled nursing settings. This is primary care work — you'll be managing chronic conditions, handling acute changes in condition, and getting to know your patients well enough to catch problems before they turn into hospital transfers.

Our group has been doing this a long time, and we built our model around one idea: patients do better when their provider actually knows them. That means real time at the bedside, not five-minute drive-bys.

What the job actually looks like

  • Rounding on a consistent panel of ALF and board and care residents, plus some SNF and home visits depending on territory and interest
  • Managing chronic disease, med changes, and acute issues on-site — helping residents and facilities avoid unnecessary ER trips when it's appropriate to treat in place
  • Doing thorough visits: history, exam, care planning, family and staff communication
  • Working alongside facility staff, plus our own psych, palliative care, and neurology teams when a patient needs more than primary care can offer
  • Keeping documentation current in our EMR (training provided)

Who we're looking for

  • Licensed NP or PA in good standing
  • Comfortable in an ALF or board and care setting, or ready to learn — we bring on new grads and seasoned providers alike, and pair new hires with a preceptor for onboarding
  • Open to SNF exposure as well, if that's of interest — we're happy to build a mixed panel for providers who want that variety
  • Background in geriatrics, internal medicine, family practice, home health, hospice, or ALF/SNF/board and care/LTC work is a plus, but not required if you're coachable and want to build this skill set
  • Solid clinical judgement and comfortable communicating with facility staff, families, and interdisciplinary teams
  • Valid driver's license and a reliable car — you'll be moving between facilities
