Care Team Role - Registered Nurse (Skilled Nursing)

Supervisor: Clinical Manager (DON)Job Purpose:

A Registered Nurse provides nursing care/services in accordance with the patientโ€™s plan of care as directed by their primary care provider. The Registered Nurse supervises Licensed Practical Nurses (LPNs) and Home Health Aides per Medicare, Medicaid regulations, as well as state regulations, if any.

Qualifications:
  • A graduate of an approved school of professional nursing who is licensed as a Registered Nurse by the state in which practicing.
  • At least one year of nursing experience preferred.
  • Home health care experience preferred.
Florida Requirements:
  • The Registered Nurse functions as the case manager for patients receiving nursing services or nursing in combination with therapy services.
  • The Registered Nurse is responsible for coordination of care, implementation of the plan of care, and maintenance of the clinical record for assigned patients.
  • The Registered Nurse provides clinical supervision of Licensed Practical Nurses, Home Health Aides (HHAs) and Certified Nursing Assistants (CNAs) involved in the patientโ€™s care, consistent with Florida law and agency policy.
Training:

Outcomes and Assessment Information Set (OASIS) training preferred.

Compensation: ย 

PRN orย Part-Time $45 - $70/Hourly Based on Experience

Responsibilities:
  • Provides comprehensive assessments (initial and ongoing) including OASIS items for Medicare and Medicaid patients.
  • Develops and implements the patientโ€™s plan of care in accordance with the patientโ€™s primary care provider.
  • Reviews the patientโ€™s medications and updates the patientโ€™s medication profile when there are new, changed, or discontinued medications.
  • Evaluates the patient or caregiver/family memberโ€™s ability to manage medications appropriately.
  • Assigns and supervises LPNs and Home Health Aides as required by Medicare/Medicaid and state regulations, if any.
  • Participates in orientation and training of new personnel.
  • Evaluates and completes competencies for Registered Nurses, LPNs, and Home Health Aides as directed by the Clinical Manager.
  • Maintains accurate and timely documentation of care/services provided.ย 
  • Notifies the primary care provider, as applicable, when there are changes in the patientโ€™s plan of care or clinical changes. ย  ย  ย  ย 
  • Communicates with other team members, as appropriate, regarding the patientโ€™s plan of care, and updates team members when there are changes.
  • Attends and participates in staff meetings, educational in-services, and committee meetings, as appropriate.
  • Participates in the on-call rotation to ensure the agency provides 24-hour nursing services.
  • Reports adverse events, incidents, and grievances/complaints (patients and/or personnel) to the Clinical Manager.
  • Provides education and information to the patient and/or family members as appropriate to their plan of care.
  • Evaluates the patientโ€™s progress and effectiveness in meeting goals and outcomes.
  • Participates in and coordinates the patientโ€™s discharge from service.
  • Participates in the agencyโ€™s Quality Assessment and Performance Improvement (QAPI) program, as appropriate.
  • Adheres to the agencyโ€™s policies and procedures.
  • Maintains confidentiality of all aspects of job performance.
  • Practices proper infection control and utilizes PPE as indicated.
  • Only performs treatments or special procedures as authorized by the Home Health Agency and deemed competent to perform by the Clinical Manager.
  • Complies with all applicable federal, state, and local laws and regulations in performance of job functions.
  • Performs other related functions as directed.ย  ย ย
