Job description

Description
The LPN or RMA provides direct patient care and clinical support within a primary care setting to ensure high-quality, efficient, and patient-centered care. Working as an integral member of the interdisciplinary healthcare team, this role supports providers through clinical workflows, pre-visit planning, patient triage (as assigned), medication processes, documentation, and care coordination while maintaining compliance with clinical, safety, and regulatory standards. The purpose of this role is to positively impact patient well-being through excellent clinical care, effective communication, and strong collaboration with the healthcare team.
Essential Functions
Morning Huddle & Pre-Visit Planning (10%)
  • Prepare Pre-Visit Planning (PVP) materials for assigned provider and care team.
  • Review and ensure PVP is completed accurately each morning.
  • Participate in daily huddles and demonstrate a positive, team-oriented approach.
Patient Triage (As Assigned) (Up to 5%)
  • Answer live patient calls for assigned care teams as directed.
  • Provide professional, clear communication and excellent customer service.
  • Collaborate with providers and team members to respond to patient concerns appropriately.

(Note: Triage responsibilities may vary based on licensure and clinic needs.)
Medication Prior Authorizations (5-10%)
  • Complete medication prior authorizations as directed.
  • Communicate with patients regarding status updates and next steps.
  • Ensure timely and accurate documentation in the EHR.
Direct Patient Care (Approximately 75%)
  • Provide direct patient care in accordance with clinic workflows and scope of practice.
  • Collaborate with providers to support efficient, high-quality patient care.
  • Administer immunizations as ordered.
  • Perform point-of-care testing as ordered.
  • Complete accurate and timely EHR documentation.
  • Maintain compliance with clinical, infection control, safety, and regulatory standards.
Service & Additional Responsibilities (5%)
  • Attend meetings, conferences, and required trainings.
  • Complete additional duties as assigned by the Clinical Services Manager or Director of Nursing and Clinical Operations.
Minimum Requirements
Education:
High School Diploma or equivalent.
Licensure/Certification (One of the Following Required):
  • Current Licensed Practical Nurse (LPN/LVN) license
    OR
  • Registered Medical Assistant (RMA) certification

Additional Requirements:
  • Current CPR certification
  • Experience working in an office or primary care clinical setting
Preferred Qualifications
  • One (1) to three (3) years of primary care experience
Working Conditions
This position operates in a climate-controlled clinical environment. Duties require sitting, standing, walking, bending, reaching, and frequent use of hands for clinical tasks. The employee must regularly lift up to 10 pounds and occasionally lift up to 25 pounds. The role requires the ability to hear, speak, and interact effectively with patients, providers, and team members.
