Physi Core Solutions is actively seeking a dedicated and passionate Physical Therapist to join our team in Springfield, Illinois. Work Environment

This role is based onsite and will involve visiting patients in their home environments, ensuring therapy is delivered in a familiar and comfortable setting. You will collaborate closely with other healthcare professionals, including Occupational and Speech Therapists, to ensure comprehensive care. The working hours are Monday through Friday, from 8:00 AM to 5:00 PM, with occasional flexibility required to accommodate patient and clinic needs.

Key Responsibilities
  • Implement individualized treatment plans tailored to the unique needs of each patient, focusing on optimizing strength, mobility, and functional outcomes.
  • Guide patients through therapeutic exercises and activities designed to promote recovery, facilitate pain relief, and enhance physical well-being.
  • Educate patients and their families about body mechanics, self-management strategies, and home exercise programs to encourage continued progress outside of therapy sessions.
  • Administer therapeutic modalities such as electrical stimulation, ultrasound, and heat/cold therapy, ensuring safety and effectiveness.
  • Closely monitor and document patient progress through objective measures such as range of motion and strength gains, contributing to data-driven treatment decisions.
  • Prepare treatment areas and ensure equipment and supplies are organized, facilitating an efficient therapy process.
  • Collaborate with an interdisciplinary team to coordinate patient care, sharing insights and contributing to comprehensive rehabilitation strategies.
  • Provide emotional support and motivation to patients, creating a positive and encouraging therapy environment.
  • Maintain the highest professional and ethical standards, ensuring patient confidentiality and respect for their privacy rights.
