• Direct comprehensive physical therapy treatment aimed at preventing disabilities and alleviating pain while enhancing muscle function and ensuring optimal performance based on patients' capabilities.
• Conduct thorough evaluations using diagnostic tests to assess clients' functional abilities, ensuring an accurate understanding of their rehabilitation needs.
• Oversee and mentor Physical Therapy Assistants in the implementation of treatment plans, ensuring compliance with regulatory standards and optimal patient care.
• Administer manual therapeutic exercises, educate patients on non-manual activities, and guide functional exercises tailored to individual rehabilitation goals.
• Provide home-based physical therapy services as prescribed by physicians, coordinating care that meets the specific needs of each client in their environment.
• Evaluate clients' home settings, offering recommendations to enhance their independence and safety as they recover.
• Maintain meticulous clinical records including initial evaluations, progress notes, and discharge summaries, ensuring timely submission for patient records.
• Engage with clients and their families, providing education on their rehabilitation programs and device utilization.
• Actively participate in interdisciplinary team meetings, providing updates on client progress and collaborating on comprehensive care strategies.

In this role, you will work with a varied patient population encompassing post-surgical recovery, chronic pain management, and rehabilitation following injury. Each client presents unique challenges, allowing you to apply your expertise in a manner that directly impacts their recovery and overall well-being.

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current Physical Therapist licensure in good standing within Illinois.
• One year of experience as a physical therapist in an acute care or home care setting.
• Current CPR certification.
• Valid driver's license and automobile insurance.

• Experience with rehabilitation for diverse patient populations, including geriatrics, athletes, and post-operative patients.
• Proficiency in utilizing assessment tools and therapy modalities to ensure tailored treatment plans.
• Previous experience in a collaborative environment, engaging with interdisciplinary teams.
