Job Title: Patient Care Manager Assistant

Position Overview:
The Patient Care Manager Assistant serves as an assistant to the agency Executive Director to assure accurate computer data entry, updated patient records, and oversight of daily agency office processes. A few position responsibilities include:
  • Assists with phone calls from field staff, physician offices, referral sources, patients, and takes messages and orders as needed.
  • Assists with phone messages and answers calls and questions in a timely manner.
  • Frequently monitors the status of incoming faxes and reports to appropriate personnel.
  • Assists with visit verification, scheduling and projection of visits, and other reports in agency systems.
  • Performs high risk patient phone calls as assigned.
  • Assists with the Interdisciplinary Group (IDG).
  • Assists the Executive Director with weekly reports.
  • Provides skilled nursing services under the supervision of RN as needed (see LVN-LPN job description).
  • Participates in Quality Assessment Performance Improvement program and the overall performance improvement plan, program, and process.
  • Performs chart audits as assigned by the Executive Director.
  • Adheres to and follows the care management process.
  • Maintains strict confidentiality of all patients.
  • Maintains competency through assigned online courses and required in-services.
  • Other appropriate tasks and special projects as assigned.
The Must Haves:
  • Current LVN/LPN licensure in state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • 1 Year previous home health experience.
