Job description

Position Summary: Tidal Basin is seeking qualified candidates for the role of Case Manager for an upcoming FEMA Individual Assistance Case Management Program (Disaster Case Management Program, or DCMP) in Baton Rouge, LA.
Disaster Case Management is a time-limited process by which a skilled Disaster Case Manager partners with a disaster-affected individual or family (Client) to plan for and achieve realistic goals for recovery following a disaster. The Disaster Case Manager serves as a primary point of contact, assisting the Client in coordinating necessary services and resources to address the client's complex disaster recovery needs to re-establish normalcy. The Disaster Case Manager reports to the Disaster Case Manager Supervisor.
Duties and Responsibilities -The position will be responsible for accurate case documentation and development of resources and referrals to individuals and families. The disaster case manager will work closely with the local committees and present case information in a confidential manner to obtain funding support for necessary unmet needs for disaster survivor recovery.
The Disaster Case Manager will be responsible for the following:
  • Perform outreach, intake, and assessments of potential clients, gather documents to establish disaster program eligibility.
  • Once established, maintain a caseload of clients, and sustain regular and required contacts within guidelines; develop, implement, and monitor a disaster Recovery Plan with each client.
  • Maintain a high standard to protect client information.
  • Provide resources and referrals to clients to meet their disaster-related unmet needs.
  • Maintain client-related data systems, including case notes and a complete system of record entries.
  • Attend meetings and training provided by Disaster Case Management Program, internal and external.
  • Attend program-related community, coalition, and committee meetings as assigned, network and advocate for clients and their unmet needs.
  • Work collaboratively with Team, Disaster Case Manager Supervisor, and Program Management to meet program goals and performance targets.
  • Exhibit a strong work ethic and a desire to adhere to strict boundaries and professional ethics in the care of others, awareness of the impact of a disaster on the community, family, and individual.
  • Conduct crisis intervention as needed.
  • Maintain a complete understanding of all applicable Program policies and requirements.
  • Other duties as required.

Experience and Education:
a) Individual shall possess 2 years' experience as a member of a state or federal recovery operation, or equivalent experience commensurate with the ability to perform the necessary duties.
b) This individual shall demonstrate an advanced knowledge of the operational and regulatory aspects of the FEMA Individual Assistance.
c) Basic knowledge of the FEMA Disaster Case Management Program is desirable.
d) Provide assistance to the applicant throughout the application process.
