Soul Saving Station Shelter is seeking a compassionate and organized Case Manager to support residents by providing direct case management services, coordinating care plans, and helping individuals work toward stability, wellness, and long-term independence.

This role is ideal for someone who is passionate about serving vulnerable populations, building strong client relationships, and helping residents navigate resources and support systems.

The Case Manager plays a vital role in advocating for residents, coordinating services, maintaining documentation, and ensuring individualized support throughout the case management process.

Responsibilities

  • Manage resident caseloads and coordinate individualized service plans
  • Monitor resident progress and evaluate case outcomes
  • Help residents work toward personal goals, wellness, and greater independence
  • Advocate for residents and assist with informed decision-making
  • Coordinate care and collaborate with outside agencies, service providers, and community resources
  • Maintain accurate case notes, incident reports, and resident documentation in Mission Tracker
  • Participate in behavioral support planning and disciplinary procedures when necessary
  • Respond appropriately to emergency situations and coordinate with law enforcement, fire, or mental health services when needed
  • Participate in team meetings and case review discussions
  • Assist with on-call coverage on a rotating monthly basis
