Job Title: Lead Diabetes EducatorDepartment: EndocrinologyLocation: Endocrine-Diabetes Care & Resource Center / Nutrition and Weight Management CenterAddress: 224 Alexander Street, Rochester, NYHours Per Week: 36 hoursSchedule: Monday to Friday 8am to 4:30pmSummary:To provide direct support to RD’s and CDE’s and assist with diabetes education of both inpatients and outpatients. Develop consistent patient care plans across RRH sites with physicians, nursing, and educators. In addition, assist with educational programs for staff to meet mandates from regulatory and accrediting agencies and hospital policies.Key Responsibilities:• Contributes to and promotes the goals of Rochester Regional Health (RRH)• Schedules, plans, or conducts education programs as needed.• Maintains and reviews appropriate teaching aids and materials to conduct education programs utilizing most recent data and research.• Evaluates learning needs for hospital employees.• Maintains and reviews appropriate teaching aids and materials to conduct education programs utilizing evidenced based medicine.• Helps maintain standards and continuity of care.• Assist with the advancement of continued DSME classes, including promotion, adherence, and the presentation of materials.• Coordinates and reviews electronic education via the learning management system.• Works closely with the RRH system LMS team and education departments to coordinate all staff education including orientation of staff.• Does Completes individual counseling as needed or required on both inpatients and outpatients.• Conducts and maintains certification of Diabetes Education Classes documenting appropriate statistics and quality measures as required by ADA or AADE.• Acts as resource person for staff and other educators in the system.• Serve as a liaison serves a to clinical and administrative team members, providing in-services on diabetes, POC bedside monitoring credentialing of staff and troubleshooting problems with blood glucose monitoring.• Assists the lab with proficiencies and determination of blood glucose monitoring selection.• Serves as a resource or delegates attendance at routine meetings as a representative of the education department.• Chairs the Education Council.• Assists with the review and update on hospital system diabetes policies regarding diabetic management and educational issues.• Maintains insulin pump and CGM programs through out the education territory.• Provides training and development for staff. Works with clinical managers of specific insulin pump manufacturers to educate on new products.• Assists with off-site educators to maintain standards of care for diabetes management across RRH.• Assists with managing the educator’s schedules as needed.• Assists in developing yearly budget, adherence to goals, and seeks financial and non-financial opportunities.• Determines the staffing plan that ensures quality care, patient and employee satisfaction within budget.• Inspires, empowers, and facilitates staff in carrying out the functions and leadership of the diabetes education programs.• Effectively communicates with staff and conducts regular staff meetings, while remaining connected to the other education departments.Preferred Qualifications:• Experience conducting health education classes.• Two years’ experience with diabetic education.• Has good verbal and written communication skills.• Is computer efficient in Microsoft office products.Minimum Qualifications:• B.S. in Nursing or Health Education or other related degree.Required Licensure/Certification:• Valid NYS RN license or NYS Registered Dietician license.• Certified Diabetic Educator• Annual BLS certification requiredEDUCATION:LICENSES / CERTIFICATIONS:PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations.PAY RANGE:$65,000.00 - $90,000.00CITY:RochesterPOSTAL CODE:14607The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
