Job description

The Case Manager is responsible for completing Intakes and providing supportive services, information and referrals, and general case management services to clients. The Case Manager is responsible for maintaining client information in accordance with agency policies and gathering and reporting outcome data in the agency client file database and reporting program metrics. Responsible for providing direct services to food pantry clients. The successful candidate will have strong interviewing skills, be able to work with diverse populations in crisis, will demonstrate compassionate and caring service provision and will be comfortable working within the mission and vision of the Catholic Charities agency.

The successful candidate will interact with prospective clients in a respectful and timely manner to help ensure delivery of appropriate program-related services and referrals. Collaboration with agencies such as the City, State and Federal agencies, Donors, community members, volunteers and Catholic Charities staff is required.

PRINCIPAL DUTIES

  • Provide direct client services including, but not limited to, client intake and eligibility determination, assessments, individualized appropriate information and referral, advocacy, and case monitoring. Maintain current list of outside service providers to provide appropriate referrals to designated clients and those who are not eligible for program services.
  • Maintain accurate and timely documentation in client database/files in compliance with agency policies and guidelines, including, intake and assessment, case notes and other documents as required by funding sources and agency policy. Ensure the proper security of client records and confidentiality in compliance with agency standards.
  • Assist Supervisor and other program staff with accurate and timely program reporting of outputs and outcomes, reports for funding agencies and ensure data is entered correctly into any and all databases and spreadsheets.
  • Process rent assistance checks on behalf of designated clients. Cultivate relationships with landlords and property management companies serving low-income families.
  • Provide administrative support to program team members in their overall mission of assisting families and individuals as may be required.
  • Collaborate with other social service agencies to coordinate client services.
  • Perform other duties as assigned.

PREFERRED QUALIFICATIONS

  • Associate degree in social work, psychology, or a related field

MINIMUM QUALIFICATIONS

  • High School Diploma or G.E.D
  • One (1) year experience in a social service setting or as an office administrator
  • One (1) year experience interacting in person and by telephone with people in need of assistance
  • Bilingual in English and Spanish
  • Valid Texas Driver's License

SKILLS REQUIREMENTS

  • Excellent written and verbal communication skills.
  • Knowledge of community resources and ability to work in diverse communities
  • Ability to follow complex oral and written directions with attention to detail.
  • Excellent organizational skills and ability to self-motivate
  • Intermediate experience in MS Office: Word, Excel, Outlook, creating publications and using the internet
  • Pleasant disposition and professional demeanor.
  • Strong interpersonal skills with the ability to build rapport and trust with diverse populations.
  • Ability to appropriately handle and maintain confidential information.
  • Able to prioritize work responsibilities, handle multiple tasks, and work in a fast-paced environment.
  • Demonstrate knowledge and understanding of professional boundaries with clients, staff and volunteers
