Home Helpers Home Care of Manassas-Gainesville is seeking a compassionate, organized, and motivated Care Manager to join our growing team! Responsibilities:
  • Coordinate caregiver schedules and client assignments
  • Assist with onboarding and training new caregivers
  • Coordinate client admissions and nursing assessments
  • Develop and maintain care plans
  • Monitor WellSky documentation and compliance
  • Maintain accurate client and caregiver records
  • Communicate with caregivers, nurses, clients, and family members
  • Follow up on missed visits and scheduling changes
  • Ensure quality care and client satisfaction
  • Maintain HIPAA compliance

Qualifications:
  • High School Diploma or GED required
  • Associate's or Bachelor's degree in Healthcare Administration, Nursing, or related field preferred
  • Home care, healthcare, or care coordination experience preferred
