Reliable Home Care Providers, Inc. is hiring a full-time or part-time Arabic-speaking Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide skilled home health visits to patients in their homes throughout Morton Grove, Niles, Skokie, and parts of Chicago.   Build your own schedule, manage your own caseload, and deliver true one-on-one care-with the autonomy of field nursing and the support of a top-performing agency behind you.  


What This Role Offers

  • $75-$85 per visit, commensurate with experience
  • Self-scheduling - build your own weekly schedule
  • Mileage reimbursement for all patient travel
  • Full-time or part-time opportunities
  • $300 sign-on bonus after 90 days

Responsibilities

  • Complete the nursing process, including assessment, planning, implementation, and evaluation, with OASIS documentation as required
  • Perform skilled nursing visits and oversee patients from admission through discharge
  • Develop and revise individualized plans of care based on patient needs
  • Coordinate care with physicians, referral sources, caregivers, and interdisciplinary team members
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), as appropriate
  • Complete all clinical documentation accurately and timely in the electronic medical record (EMR)

  • Morton Grove
  • Niles
  • Skokie
  • Parts of Chicago
  • Surrounding  areas

Requirements

Current, active Illinois RN license in good standing

Fluent in Arabic and English (required) 

Current CPR/BLS certification

Valid driver's license, reliable vehicle, and auto insurance (required for travel between patient homes)

Prior nursing experience; home health experience and OASIS familiarity preferred

Comfort with clinical documentation software; WellSky Home Health EMR experience is a plus, but not required

Strong assessment, problem-solving, and communication skills

Ability to perform the essential functions of field nursing, with or without reasonable accommodation
