Job description

Position: Aftercare Case Management Specialist (Hybrid)
Salary: $41,000.00 - $49,000.00 annually
(Starting salary depends on education and experience)
Job Type: Full Time
RESPONSIBILITIES: The Aftercare Case Manager reports to the Manager of Aftercare, however, has a high degree of autonomy in carrying out aftercare case management services for cases as required by contract/licensing/funding body/accreditation requirements. Using advanced knowledge in the area of child welfare, the Aftercare Case Manager exercises discretion and independent judgment in making decisions on the overall delivery of services to children and families on their caseload. The Aftercare Case Manager will coordinate and partner with the family, the family’s natural supports, and members of the assigned team to safely maintain children in their homes and assist the family with the natural adjustment that comes following reintegration. The Aftercare Case Manager will also provide case management services for adoption cases as required by contract/licensing/funding body/accreditation requirements. The Aftercare Case Manager will demonstrate creative service delivery, strong time management skills, and, excellent decision-making ability.
QUALIFICATIONS: This position requires a Bachelor’s degree in Social Work or other Human service-related field and at least three years of work experience with children and families. The preferred candidate will be Licensed by the Behavioral Sciences Regulatory Board to practice in Kansas (LMSW, LSCSW, Professional Counselor, etc.) with a Master’s degree from an accredited university in Social Work or a related field, which includes the following: Community Counseling, Human Development, Child and Family Development, Applied Family and Youth Studies, Public Health, Health Sciences, Trauma Studies, Sociology/Social Services, Substance Abuse/Addictions, Education/Early Childhood, or Psychology. Must be at least 21 years of age and pass a background check, physical, and drug screening. This position also requires a valid driver’s license and proof of current vehicle insurance.
