Job Summary
Our client is seeking an IT Application Support Analyst to provide primary care to a patient panel managing chronic kidney disease and multi-system comorbidities.
Key Responsibilities
  • Provide primary care to a patient panel managing chronic kidney disease and multi-system comorbidities.
  • Complete 5 visits per day / 20 visits per week.
  • 80% of visits must be in-home; remaining 20% can be telehealth/virtual.
  • Manage tight turnaround metrics for note closures and hospital transition visits.
  • Maintain the engagement of a maximum of 200 engaged patients, assisted by the support pod.

Qualifications
  • Minimum of 3 to 5 years of experience as a practicing APP.
  • Core Primary Care background.
  • Strict policy: No new graduates.
  • Experience within a Value-Based Care model is preferred, especially prior professional experience conducting home health/in-home medical visits.

Experience
  • 3 to 5 years of experience in primary care.

Skills
  • Strong primary care skills.
  • Experience with home health/in-home medical visits.
