We are recruiting multiple roles within the malignant hematology oncology subspecialty. See below for details:
1) Clinical Pharmacy Specialist, Inpatient Malignant Hematology / Transplant & Cellular Therapy
The Clinical Pharmacy Specialist for Inpatient Malignant Hematology / Transplant & Cellular Therapy will serve as an integral member of a multidisciplinary hematology care team, primarily within the inpatient setting with an ambulatory component. This position offers a dynamic and varied practice experience, rotating between the inpatient acute leukemia/allogeneic transplant service (approximately four weeks), the inpatient multiple myeloma, lymphoma, autologous transplant, and cellular therapy service (approximately four weeks), and our outpatient postdischarge transitionsofcare malignant hematology clinic (approximately four weeks). This model provides professional variety, broadens clinical exposure, and supports continuity of care as patients transition across settings.
You will collaborate closely with physicians, nursing staff, and allied health professionals to optimize complex treatment regimens, ensure safe and effective medication use, and support evidencebased therapeutic decisions. Additional responsibilities include facilitating smooth transitions of care and contributing to qualityimprovement and practicedevelopment initiatives. This role also provides opportunities to precept learners-including APPE students and PGY1 and PGY2 pharmacy residents-helping to support the growth and development of future oncology pharmacists.
2) Clinical Pharmacy Specialist, Ambulatory Malignant Hematology
The Clinical Pharmacy Specialist for Ambulatory Malignant Hematology will join a highly collaborative outpatient care team dedicated to delivering comprehensive, patientcentered pharmaceutical care. In this role, you will provide direct ambulatory oncology services, including therapy management, patient education, toxicity assessment and monitoring, and optimization of supportive care. Two positions are available: one focused primarily on leukemia clinics, and the other focused primarily on lymphoma clinics.
You will work closely with physicians, advanced practice providers, and nursing colleagues to support individualized treatment plans, advance evidencebased practice, and contribute to qualityimprovement and practicedevelopment initiatives. This role also includes opportunities to precept learners, including APPE students and PGY1 and PGY2 pharmacy residents, helping to support the growth and development of future oncology pharmacists.
A Brief Overview
The Clinical Pharmacy Specialist provides a wide range of services in their area of expertise. These services may include, but are not limited to, rounding with physicians and other healthcare providers to drive evidence based and institution guideline directed medication therapy management, active participation in the training of pharmacy students and pharmacy residents, assist with policy and guideline development, and active participation in local and system level committees and workgroups in their area of expertise.
What You Will Do
- Provide Clinical Pharmacy Services (drug information, drug therapy monitoring, patient specific consultations, healthcare staff education, pharmacoepidemiology and pharmacoeconomic evaluations in selected patient populations)
- Investigate/research/report adverse drug reactions
- Participate in Medication Safety and Therapeutics (MS&T) Committee process: evaluate requests for new drug usage and old formulary drug relevance and review therapeutic classes for relevance, assist in maintaining and updating the system formulary
- Serve as an informational and patient problem solving resource for Clinical Pharmacists and other healthcare professionals
- Provide continuing education to Clinical Pharmacist staff
- Documentation of clinical activities
- Serve as a preceptor for pharmacy residents and pharmacy students
- Actively participate in local and system committees and workgroups pertaining to the Clinical Pharmacy Specialist's area of expertise
Additional Responsibilities

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Education
- Doctorate Degree in Pharmacy with completion of an ASHP accredited residency/residencies or 3-5 years of experience in the practice area. (Preferred)
Work Experience
- less than 1 year Experience as a pharmacist with training in drug therapy of diseases including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients, training in cost analysis/cost evaluation techniques (Preferred)
Knowledge, Skills, & Abilities
- Computer systems skills in spreadsheet, database and word processing (Preferred proficiency)
- Must demonstrate highly effective written and oral communication skills (Required proficiency)
