Location: Columbus and Moraine, Ohio
Position Type: Full-Time / Part-Time / Per Diem
Pay Range: $80,000 – $140,000 per year (based on productivity and visit volume)

Guardian Angel Home Care is seeking compassionate and motivated Registered Nurses (RN) to provide skilled nursing care to patients in their homes. Our nurses play a vital role in helping patients recover, maintain independence, and receive high-quality care in the comfort of their own homes.

The Home Health Registered Nurse provides skilled nursing services to patients in their homes in accordance with physician orders and the patient’s individualized Plan of Care.

The RN is responsible for performing patient assessments, coordinating care, educating patients and families, and ensuring compliance with Medicare, CMS, CHAP, and state regulatory requirements.

This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field.

• Perform comprehensive patient assessments including OASIS assessments when required
• Develop, implement, and update the Plan of Care in collaboration with physicians and the interdisciplinary team
• Provide skilled nursing treatments and interventions as ordered by the physician
• Monitor patient progress and document outcomes of care

• Communicate patient status and changes to physicians and the care team
• Coordinate services with therapists, aides, and other healthcare professionals
• Ensure timely follow-up on physician orders, lab results, and care updates

• Maintain accurate and timely clinical documentation
• Ensure compliance with CMS, Medicare, CHAP, and agency standards
• Participate in clinical reviews, quality assurance, and chart audits when requested

• Educate patients and caregivers on disease management, medications, and safety
• Provide guidance on preventative care and recovery strategies

• Supervise LPN/LVN staff and home health aides when appropriate
• Participate in meetings, training programs, and agency in-services
• Support quality improvement initiatives and best practices in home health care

Required

• Active Registered Nurse (RN) license in the state of employment
• Graduate of an accredited School of Nursing
• Valid driver’s license and reliable transportation
• Current CPR certification

Preferred

• Previous Home Health experience
• Experience with OASIS documentation
• Strong assessment and critical thinking skills

• Excellent clinical assessment and nursing judgment
• Strong communication and documentation skills
• Ability to work independently and manage time effectively
• Compassionate approach to patient care

• Ability to travel to patient homes
• Ability to lift up to 50 pounds occasionally
• Ability to stand, bend, reach, and move during patient care activities

This position is primarily field-based, providing care directly in patients’ homes. The RN will travel between patient locations within an assigned territory.
