Responsibilities

Therapist- IOP

 Horizon Health is seeking a Therapist for our BRAND NEW IOP mental health clinic called Thousand Branches Wellness in Mesa, AZ to join our team! This clinical position is responsible for day-to-day clinical operations of the assigned location. Responsibilities include but are not limited to clinical assessment, group therapy, individual therapy, interdisciplinary treatment planning and other clinical interventions as deemed necessary. Clinical operations will be offered both in person and virtually.

Responsibilities:

  • Curriculum development and implementation
  • Referral management, assessments and direct client contact (individual, group and family)
  • Interdisciplinary treatment team planning
  • Maintain confidentiality through good judgement
  • Implement and manage systems to ensure effective communication with clients, families, referral sources and other relevant entities
  • Treatment, discharge and safety planning
  • Maintain active licensure and participate in payer credentialing
  • Active participation in clinic consultation, supervision and collaboration
  • Thorough and timely clinical documentation
  • Active engagement in performance improvement
  • Maintain a safe, respectful and collaborative work environment
  • Organize and prioritize workloads to meet deadlines
  • Monitor and manage client satisfaction through standardized measures 
  • Other duties as assigned
