Make a Difference for Arizona’s Special Needs Community!


Are you someone who cares deeply about people with developmental disabilities? Do you want to do work that matters — and still enjoy the flexibility of independent, on-the-road work?

LifeTree, a DDD agency, is hiring a Home Visit Field Representative located in Central, North OR West Phoenix to conduct brief, supportive check-ins with children and adults who have special needs.

These in-home visits are not caregiving — they’re compliance and support visits to make sure clients are safe, supported, and receiving the care they deserve.


You’ll be the friendly, observant, and professional face of our agency in the community.


What You’ll Be Doing:

  • Visit individuals with developmental disabilities in their homes to ensure services are being delivered correctly
  • Observe the home environment and support situation (no physical care required)
  • Interact respectfully and professionally with both individuals and their caregivers
  • Complete brief, standardized documentation after each visit using your phone or tablet
  • Drive to multiple homes daily (10–15 min visits), coordinated by our admin Booking Team
  • Must be comfortable to drive to rural areas if needed.


You’ll be on the road about 90% of the time, working independently and making a visible impact
