Job description

Job Summary

This is a full-time professional position. This position is responsible for coordinating nursing and hospice aide staff to ensure day-to-day continuity of care for all patients on service and ensure accountability of nursing care consistent with the agency’s mission statement and regulatory systems. This position supervises the assigned shift Hospice Home RNs, LPNS, and hospice aides. May intermittently include day to day supervision of all interdisciplinary team members. The schedule for this position is Monday-Friday 10:30pm-7:00am; weekends as needed; holidays as scheduled.

Responsibilities what you do

  • Coordinate interviewing/hiring and orientation of new clinical staff.
  • Support and assist with training of new staff
  • Support and assist with medication delivery from pharmacy
  • Supervise, direct, prioritize and coordinate delivery of patient care, on the assigned shift, for Hospice Home patients.
  • Ensure all admissions are assigned appropriately to clinicians
  • Create a positive environment, fostering collaboration and a supportive atmosphere
  • Demonstrate strong communication, problem solving, organizational and delegation skills
  • Perform chart audits weekly to ensure compliance with documentation requirements.
  • Coordinate and problem-solve patient care issues with patients, families, physicians, agency staff and other health care providers.
  • Ensure staff documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements.
  • Manage various personnel actions including, but not limited to hiring, orientation, performance appraisals, promotions and scheduling; and counsel and conduct formal discipline of clinical staff.
  • Develop and implement processes to enhance clinical operations and practices for ongoing quality improvement.
  • Oversee medication room operations and regulatory compliance related to narcotics and other medications.
  • Oversee clinical compliance with all regulatory and accreditation needs of the agency.
  • Review, update, and implement policies, procedures and clinical processes as appropriate.
  • Identify annual competency and staff development needs, in conjunction with PI and Education, ensuring all clinical staff are compliant with identified competencies.
  • Covers Management Back Up (MBU) as assigned
  • Covers at least one holiday per year as assigned
  • Attend seminars/workshops as appropriate.
  • Perform other duties as assigned by supervisor.
  • Professionally manages time, people, and technology.

Requirements what you need

  • RN with current N.C. license.
  • Two years of previous supervisory experience preferred.
  • Current CPR certification.
  • Thorough understanding of Hospice clinical interventions and protocols.
  • Knowledgeable about reimbursement sources, Medicare/Medicaid regulations, licensing laws and accreditation standards.
  • Excellent written and verbal communication skills.
  • Strong team building skills
  • Strong leadership abilities.
  • Competent in use of computer, able to learn and teach use of new software programs.
  • Must have access to transportation.
  • May be required to lift 30 lbs.
  • Computer literacy with Windows programs. Ability to learn Allscripts programs.
  • Current unrestricted NC driver’s license.
  • Current professional license
