Opportunity:
RN - Home Health Nurse
Job Details:
• Work 12-hour shifts on Saturday and Sunday from 8:00 AM to 8:00 PM
• 24 hours per week guaranteed on a 10-week contract
• Service the Racine, Kenosha, and Mt. Pleasant territory
• Home Hospice position requiring travel to patients' homes
• May occasionally float slightly outside of assigned territory, never more than one hour from home territory
• Minimum 2 years of Home Health or Home Hospice experience required; first-time travelers accepted with case management experience in the home setting
• Patient types include failure to thrive, advanced cancers, end stage COPD, end of life, terminally ill, and end stage disease processes
• Patient ratio: 0-6 patients depending on call-ins
• Attend after-hour visits called into triage line including PRN visits for symptom management, falls, and death visits; some issues may be resolved via phone
• Must be comfortable performing focused assessments and critical thinking, investigating patient agitation issues, exchanging Foley catheters, troubleshooting feeding tubes, wound care, post-fall assessments, death visits, and administering narcotic pain medications
• Must be comfortable identifying potential drug diversions in the home
• Lead RN available 24/7 for support; must update Lead RN after each visit and provide report
• EMR: Epic (Rover), Remote Client
• IV Pump: Sapphire (rare in home setting)
• Required certifications and credentials: BLS, active driver's license, State of Wisconsin RN license, own vehicle, and auto insurance
• Badge, laptop, cell phone, and trunk kit provided upon arrival
• Scrub color: Navy blue
