Job description

Position Overview

Home Health Physical Therapist Signature Healthcare at Home is hiring a fulltime Physical Therapist to serve the Northeast Portland Metropolitan Area, including neighborhoods such as Alberta, Hollywood, Parkrose, Laurelhurst, Gateway, and surrounding communities. This role is ideal for PTs who thrive in dynamic, communitybased care environments.

Key Responsibilities

PT Responsibilities

  • Conduct homebased physical therapy evaluations.

  • Develop individualized treatment plans focused on functional mobility and independence.

  • Deliver therapeutic interventions including gait training, strengthening, balance work, and homeexercise program development.

  • Perform homesafety assessments and recommend appropriate DME.

  • Manage a fieldbased caseload with efficiency and autonomy.

  • Collaborate with interdisciplinary team members to ensure coordinated care.

  • Maintain accurate documentation in accordance with regulatory standards.

  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Schedule

FullTime Schedule

  • Monday-Friday

  • Normal business hours

  • Strong interdisciplinary support

Qualifications

PT Requirements

  • Current Oregon Physical Therapist license (or ability to obtain).

  • One year of PT experience preferred; homehealth experience a plus.

  • Strong communication, assessment, and clinical reasoning skills.

  • Ability to work independently in a fieldbased environment.

  • Reliable transportation and valid driver's license.
