Job Summary

Altra Home Health is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team. In this fulfilling role, you will provide individualized, high-quality care to both pediatric and adult patients in their homes. Many of our patients have complex medical needs, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and, at times, require tracheostomy or ventilator support. You will play a key role in promoting safety, comfort, and quality of life, allowing patients to receive the care they need in the place they feel most at ease—home.

Responsibilities

  • Perform thorough head-to-toe assessments and follow individualized plans of care.

  • Provide care that may include G-tube feedings, medication administration, wound care, trach care, and ventilator support.

  • Care for patients with professionalism and empathy.

  • Educate patients and caregivers on disease management, emergency procedures, and therapeutic techniques.

  • Collaborate closely with physicians, therapists, and other team members to optimize care.

  • Document all care activities accurately and promptly in compliance with agency and state requirements.

  • Maintain rigorous safety, infection control, and privacy standards.

Shift Information

  • Shift durations range from 4 to 12 hours, depending on patient needs and nurse availability.

  • Flexible scheduling options available for daytime, evening, and weekend shifts

Qualifications

  • Active LPN license in Indiana or an active compact LPN license
