Responsibilities
- Conduct comprehensive UAS-NY assessments for geriatric patients in home and community settings.
- Evaluate members’ physical health, cognitive status, psychosocial needs, functional abilities, and support systems.
- Develop and recommend appropriate plans of care based on assessment findings.
- Identify risks, safety concerns, and barriers to care while coordinating appropriate interventions and referrals.
- Collaborate with interdisciplinary teams, caregivers, physicians, social workers, and care managers to ensure continuity of care.
- Educate patients and families regarding available community resources, long-term care services, and healthcare options.
- Maintain accurate, timely, and compliant clinical documentation within electronic medical record systems.
- Ensure compliance with all state regulations, Medicaid guidelines, and organizational policies.
- Participate in case conferences, quality improvement initiatives, and ongoing training programs.
- Travel throughout assigned territories to complete member assessments.
Qualifications
- Active and unrestricted New York State Registered Nurse (RN) license required.
- Bachelor’s Degree in Nursing (BSN) preferred.
- Previous experience completing UAS-NY assessments strongly preferred.
- Experience working with the geriatric population, managed care, home care, hospice, or long-term care required.
- Strong understanding of chronic disease management, community resources, and elderly care services.
- Excellent assessment, critical thinking, and documentation skills.
- Ability to work independently and manage a flexible schedule.
- Proficiency with electronic medical records and computer systems.
- Valid driver’s license and reliable transportation may be required.
Preferred Skills
- Bilingual abilities are a plus.
- Knowledge of MLTC and Medicaid regulations.
- Strong interpersonal and patient advocacy skills.
- Ability to build rapport with elderly patients and caregivers from diverse backgrounds.
