Responsibilities

The After Hours Support Specialist will provide supportive services for active Supportive Housing participants after regular business hours to maintain housing that is consistent with the client's individualized housing goals developed in collaboration with the Supportive Housing Caseworker. In addition to their case worker, clients will have access to the After Hours Support Specialist for assistance with any additional needs that include essential life skills and crisis intervention.

The After Hours Specialists will provide professional, skilled and consistent after hours support services to active Supportive Housing clients including but not limited to, the coordination and delivery of services related to crisis intervention, grocery shopping, transportation to appointments, housekeeping, and basic daily living skills. Work in conjunction with the Supportive Housing Caseworkers to ensure consistency of service delivery between after-hours support, and regular business hours, and to maintain housing and all individualized housing goals. Research and refer clients to community resources and other organizations, and network with community service providers. Complete all necessary reporting forms for government agencies and funding sources (UCR's, progress notes, referrals, and statistics). Maintain client case files. Maintain appropriate communication with caseworkers, supervisors and other funding sources, notifying appropriate parties of any safety issues in a timely manner. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures. Perform additional duties related to housing individuals and helping them remain housed, including but not limited to furnishing and supplying apartments.

Pay for this position is $19.00/ hour

Hours for this position is: Mon, Tues, and Fri - 2pm to 10pm and Sat and Sun are 9am to 5pm

Qualifications

Education and/or Experience Required at Entry:

  • Bachelor's Degree in Human Services or related field preferred. High school diploma with three years relevant experience working with individuals with substance abuse/ mental health concerns
  • Experience with crisis intervention and supports preferred;
  • candidates with lived experience are preferred, and encouraged to apply.

Licenses/Certificate/Clearances:

  • Valid N.Y.S. Drivers License,
  • Registered & Insured Vehicle
  • N.Y.S. Child Abuse Registry clearance.

Skills/Abilities:

  • Basic computer skills.
  • Creative problem solving.
  • Negotiation skills.
