American Medical Staffing is seeking a local contract nurse RN Case Management for a local contract nursing job in Valley, Alabama.

Job Title: RN: Case Manager
Location: Valley, AL
Setting: Home Health
Pay Range: $49.08 - $51.53 per hour
Schedule: 40 hours per week (Mon-Fri 8 am - 5 pm ET, On-call 1 weekend per month)
Contract Length: Discuss with Recruiter
Candidate Type: Local candidates only – travel stipend not available

Requirements:

  • 2+ years of experience as an RN (Case Management preferred)
  • Qualified applicants MUST have an Alabama license or a compact license accepted
  • Qualified applicants MUST have current, unrestricted RN license in Alabama
  • Must have Associates or Bachelor’s Degree in Nursing or related field
  • Must have Case Management certification
  • Ability to analyze complex medical information and make rational decisions
  • Valid RN license

Preferred Skills:

  • Home Health experience
  • HCHB, Oasis experience
  • Wound care, PICC line, IV’s

Responsibilities:
As a healthcare professional in a home health setting, your key responsibilities include:

  • Perform comprehensive assessment of high-risk members (RN)
  • Collaborate with primary care providers to develop and implement individualized care plans with specific interventions
  • Address and continuously reassess appropriate levels of care to manage costs and patient safety
  • Put non-medical support systems in place, such as housing and transportation assistance, to comply with treatment plans
  • Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
  • Engage specialty resources as needed to achieve optimal patient care outcomes
  • Keep detailed records of clinical, functional, and fiscal outcomes during care management
  • Identify opportunities for health promotion and illness prevention
  • Act quickly to prevent adverse patient outcomes, acting as an advocate to protect patient privacy and confidentiality
