In the heart of The Dalles, Oregon, Adventist Health Columbia Gorge has served the Columbia River Gorge community since 1901. We are comprised of a 49-bed hospital, various medical offices, home health, a birthing center and a vast scope of services located throughout The Dalles and the surrounding areas. The Dalles is known for its picturesque views of the Columbia River, local charm and delicious downtown restaurants and bakeries. There is something for everyone in this scenic community.

Job Summary:

Delivers coordinated nursing care for patients in their place of residence according to established standards of care and the nursing process. Supervises and directs the activities of various levels of assigned nursing staff, and coordinates care with other disciplines while utilizing critical thinking, professional and supervisory discretion, and independent judgment. This position is represented by ONA.

Job Requirements:

Education and Work Experience:

  • Bachelor's Degree in Nursing (BSN): Preferred
  • One year of professional nursing experience: Required
  • Acute care facility or home health experience: Preferred

Licenses/Certifications:

  • Oregon Registered Nurse (RN) licensure: Required
  • Washington Registered Nurse (RN) licensure: Required within 90 days of hire
  • Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required
  • Valid Driver's License (DL) and must be at least 21 years of age or older: Required
  • OR-RN: Required
  • WA-RN: Required

Facility Specific License/Certifications:

  • AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR: Required
  • Auto Insurance (AUTO): Required

Essential Functions:

  • Collects relevant data pertinent to the patient's health or situation. Analyzes the assessment data in determining diagnosis and care issues. Develops a plan that prescribes interventions to attain outcomes. Implements the plan, coordinates care delivery, and employs strategies to promote health and a safe environment. Evaluates progress toward attaining outcomes. Identifies outcomes for the patient or the patient's situation.
  • Collaborates with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, humane, and caring environment. Provides learning opportunities for patients/family members and team members. Directly provides health information to patients, families, and treatment team.
  • Assesses and documents patient/family learning needs, implements teaching resources, and assesses and documents patient/family understanding. Completes all Oasis documentation accurately and completely.
  • Delegates appropriately and coordinates duties of healthcare team members.
  • Performs other job-related duties as assigned.
