Advanced Practice Provider/Nurse Practitioner

Matter Health delivers primary and preventive healthcare directly to older adults living in public housing communities.

We are seeking an Advanced Practice Provider/Nurse Practitioner to support older adults in Springfield, MA. This role may be full-time or part-time depending on market needs.

What You'll Do
  • Provide longitudinal primary and preventive care for older adult patient populations
  • Manage and grow a dedicated patient panel within assigned communities
  • Deliver patient-centered, relationship-driven care directly in community settings
  • Collaborate closely with interdisciplinary clinical and operational teams
  • Support preventative care, chronic disease management, and care coordination initiatives
  • Utilize telehealth and in-person care delivery models to improve healthcare accessibility
  • Maintain accurate and timely EHR documentation and coding specificity
Qualifications
  • Active, unrestricted Nurse Practitioner or Physician Associate license
  • Board certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) or PA (NCCPA)
  • Active DEA license and controlled substance registration where applicable
  • 3+ years of clinical experience as an APP, preferably with aging or medically complex populations
  • Basic Life Support (BLS) certification
  • Valid driver’s license and reliable transportation (car stipend provided)
  • Experience with EHR documentation (athenahealth experience preferred)
  • Commitment to equitable, patient-centered care
Strongly Preferred
  • Background in family medicine, internal medicine, adult medicine, or geriatrics
  • Experience in value-based care environments
  • Experience providing care in community-based, in-home, or non-traditional healthcare settings
  • Familiarity with MEAT documentation and chronic condition coding specificity
  • Telehealth experience
