Job description

Job Title: Registered Nurse – Case Manager
Location: Mission Hills, CA 91345
Duration: 13 weeks
Shift: 8:00 AM – 4:30 PM (5x8; Every Other Weekend)
Pay Rate: $68/hr (W2) | $73/hr (1099)

Job Summary:
We are seeking a highly skilled Registered Nurse Case Manager to support the Case Management department.
The RN Case Manager plays a critical role in coordinating patient care, facilitating discharges, and ensuring effective care transitions.
Candidates must bring at least 2 years of acute care hospital case management experience with a strong focus on care coordination and discharge planning.

Key Responsibilities:
  • Perform patient assessments to determine appropriate levels of care
  • Coordinate care and discharge planning in collaboration with interdisciplinary teams
  • Independently arrange DME, SNF placement, ambulance transport, and other services
  • Conduct family meetings and contribute to treatment plan development
  • Utilize InterQual criteria for admission reviews (ED only)
  • Work with HMOs, IPAs, and understand utilization review processes
  • Support patients with complex social needs (e.g., mental health, homelessness, substance use)
  • Navigate advanced directives, POLST, and protective services processes

Required Experience & Skills:
  • Minimum 2 years of acute care hospital case management experience
  • Proven ability to independently manage discharge planning and care coordination
  • Proficient in Microsoft Office and electronic documentation systems
  • Familiarity with InterQual admission criteria and utilization review
  • Knowledge of community resources and continuum of care services

Special Equipment/Software:
  • Electronic medical record (EMR) systems (computerized documentation required)
  • Microsoft Office Suite

Certifications:
  • Current RN license (California) – Required
  • BLS – Preferred

Notes:
  • Utilization Review knowledge is a hard requirement
  • Block scheduling is not available
  • Must be able to work independently and adapt to fast-paced environments
