  • Location: San Jose, CA (Dual Locations – Moorpark Ave & Jackson Ave)
  • Job Type: Full-Time | Permanent | Direct Hire
  • Pay: $55.21 – $64.50 per hour (Based on experience) + Benefits
  • Schedule: Monday–Friday | 8:00 AM–5:00 PM


About the Role

We are seeking an experienced Registered Nurse (RN) to join a Primary Care/Outpatient clinic in San Jose. The ideal candidate will have recent hands-on experience in a primary care setting and be passionate about delivering high-quality, patient-centered care to diverse populations.

Please Note: This position requires recent Primary Care RN experience. Candidates without direct primary care experience will not be considered.


-> Responsibilities

  • Provide direct nursing care in a primary care outpatient setting.
  • Triage patient phone calls and walk-ins, assessing symptoms and determining appropriate care.
  • Assist providers with patient assessments, treatments, and clinical procedures.
  • Review laboratory and diagnostic results and coordinate appropriate follow-up care.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Educate patients on chronic disease management, preventive care, medications, and healthy lifestyle practices.
  • Coordinate referrals to specialists and other healthcare services.
  • Maintain accurate documentation in the Electronic Health Record (EHR).
  • Collaborate with physicians, behavioral health providers, care coordinators, and other interdisciplinary team members.
  • Ensure compliance with HIPAA, clinical protocols, and organizational policies.
  • Support quality improvement initiatives and perform other clinical duties as assigned.


-> Required Qualifications

  • Active California Registered Nurse (RN) license.
  • Bachelor of Science in Nursing (BSN) required.
  • Minimum of 2 years of recent RN experience in a Primary Care clinic (required).
  • Current BLS certification.
  • Experience with patient triage, preventive care, chronic disease management, care coordination, and patient education.
  • Proficiency with Electronic Health Records (Epic experience preferred).
  • Strong clinical judgment, communication, and organizational skills.

Preferred Qualifications

  • ACLS and PALS certifications.
  • Bilingual in Spanish, Mandarin, or Vietnamese.
