Job description

Northwestern Counseling & Support Services is seeking compassionate and dedicated individuals to join our greater Adult Services Case Management Team. These case management positions serve adults with various mental health diagnoses. We are seeking individuals who are person-centered and passionate about working in the mental health field. These positions serve individuals with varying high acuity needs who may intersect with criminal justice, legal, and substance use systems. These positions are essential in supporting individuals to gain stability, address unmet needs, and to lead independent and fulfilling lives within their communities.

Key Responsibilities:

  • Providing services across various community-based settings, focusing on symptom management, daily living activities, supportive counseling, social and interpersonal skills development, money management, and advocacy.
  • Ability to build rapport with individuals who may be resistant to treatment engagement.
  • Maintain a positive attitude, work well within a team, and stay organized to manage caseloads effectively.
  • Collaboration and care coordination with community resources and agencies, as well as with internal or external service providers.ย 
  • Develop and oversee treatment plans, service coordination, crisis intervention, and representative payee services.

Qualifications:

  • Reliable transportation with the capacity to transport clients.
  • Bachelorโ€™s degree is preferred, but an associate degree in human services or relevant experience will be considered.
  • Experience in mental health case management is a plus, but we are willing to train the right candidate.
