Job description
We are seeking a nurse to support an exciting new initiative at Rex Hospital. To better serve our stroke patients, we are developing a grant-funded post-stroke clinic that will provide timely follow-up care and connect patients with social resources essential for optimal health outcomes. This is currently a part-time position (24 hrs/week), contingent on continued grant funding for two years. Upon successful completion of this initiative, the project team's goal is to establish a permanent clinic with ongoing employment opportunities beyond the grant period.
The Nurse Coordinator will provide clinical assistance to stroke clinic patients and providers, and will identify and coordinate resources based on social determinants of health screenings. Working as part of the grant project team, the Nurse Coordinator will ensure all grant requirements are met, including data collection, documentation, and clinical processes. Additionally, this role will contribute to the operational implementation and sustainability of the post-stroke clinic.
Responsibilities include coordinating clinical assignments, serving as a clinical and management resource, assisting in the development and implementation of clinical policies and procedures, providing direct patient care, maintaining adequate inventory of clinical supplies and equipment, adhering to the American Nurses Association Code of Ethics, and assisting in the development and implementation of an orientation plan for new staff.
Education Requirements: Nursing Diploma, Associate Degree (ADN) or Bachelor's Degree (BSN). Licensure/Certification Requirements: BLS certification required. Licensed to practice as RN in North Carolina. Professional Experience Requirements: Two (2) years (RN/LPN) nursing experience with at least one (1) year as a RN in a medical office or outpatient setting preferred. Prefer previous supervisory experience. Knowledge/Skills/and Abilities Requirements: Language Skills: Ability to read, analyze, and interpret clinical information and technical procedures. Ability to document clinical information. Ability to effectively communicate information and respond to questions from patients, physicians, family members and other Rex staff. Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions.
