ACT Supervisor -Case Manager
Position: ACT Team Manager (Full-Time)
Location: Tyler, Texas
Are you a dynamic clinical leader ready to make a lasting impact in the lives of individuals living with serious mental illness and co-occurring substance use disorders? Join our Assertive Community Treatment (ACT) team in Tyler and lead a mission-driven group of professionals dedicated to transforming lives through compassionate, community-based care.
About the Role
As our ACT Team Manager, you will direct the day-to-day clinical operations of a multidisciplinary team providing intensive, wraparound services to individuals in need of high-level community support. This is a leadership role for someone who thrives in a fast-paced environment, values collaboration, and is passionate about recovery-oriented care.
You will play a key role in shaping clinical excellence, mentoring staff, and ensuring that every consumer receives individualized, responsive treatment.
What You'll Do
  • Provide regular group and individual clinical supervision to ACT team members
  • Lead daily team meetings and treatment planning meetings
  • Schedule staff work hours to ensure appropriate service coverage
  • Oversee and coordinate all consumer admissions and assessments
  • Assign consumers to ACT case managers based on clinical needs
  • Continuously evaluate consumers and coordinate treatment planning according to daily needs
  • Supervise medical record documentation to ensure quality, compliance, and accuracy
  • Maintain a small caseload (no more than 5 consumers) to remain clinically engaged
  • Ensure adherence to best practices in mental health and substance use disorder treatment
Qualifications
Required:
  • Bachelor's degree in a human services field
  • Eligible for QMHP-CS designation
  • Experience in the mental health and substance use disorder field
  • Current Texas driver's license
  • Ability to meet insurance criteria
  • Ability to pass all applicable training requirements
Preferred:
  • Master's degree in a human services field (unofficial transcript must accompany application)
  • Current licensure as LPC, LCSW, LPC-I, or LMSW
