Our Client is a Growing, Patient-Centered Primary Care Practice Located in DeBary, Florida. We are Committed to Providing High-Quality, Compassionate Healthcare with a Strong Focus on Preventive Medicine, Chronic Disease Management, and Improving Health Outcomes for Medicare Advantage Patients.

The Position:

Our Client is Actively Seeking an Experienced Primary Care Physician who is Fluent in Both English and Spanish and has Extensive Experience Caring for Medicare Advantage Patients.

The Ideal Candidate is Passionate About Primary Care, Value-Based Medicine, and Delivering Exceptional Patient Care While Helping Improve Quality Metrics, Patient Satisfaction, and Overall Healthcare Outcomes.

We Are Specifically Looking for a Physician with Extensive Experience Caring for the Medicare Advantage Population and a Thorough Understanding of Value-Based Care.

The Ideal Candidate Should Have a Proven Track Record of Working with Medicare Advantage Patients and Be Well-Versed in HEDIS Quality Measures, Medicare Risk Adjustment (MRA), RAF Score Optimization, Chronic Disease Management, Preventive Care, and Annual Wellness Visits (AWVs). Experience Accurately Documenting Chronic Conditions, Improving Quality Metrics, Closing Care Gaps, and Collaborating Within a Value-Based Care Model is Essential.

Requirements:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an Accredited Medical School
  • Current, Unrestricted Florida Medical License
  • ACN Accepted
  • Current DEA Registration
  • Current BLS and ACLS Certifications
  • Fluent in Both English and Spanish
  • Minimum of Three (3) Years of Experience in Primary Care, Family Medicine, Internal Medicine, or Geriatric Medicine Preferred
  • Extensive Knowledge of Medicare Advantage Plans, HEDIS Measures, Medicare Risk Adjustment (MRA), RAF Score Optimization, and Value-Based Care
  • Strong Clinical, Communication, and Organizational Skills
  • Experience Utilizing eClinicalWorks (eCW) Electronic Medical Records Preferred

Responsibilities:

  • Full-Time Monday – Friday | 8:00 AM – 5:00 PM
  • No Nights
  • No Weekends
  • Provide Comprehensive Primary Care Services to Adult and Geriatric Patients
  • Perform Annual Wellness Visits (AWVs), Routine Physicals, and Preventive Health Screenings
  • Diagnose and Manage Acute and Chronic Medical Conditions Including Diabetes, Hypertension, COPD, Congestive Heart Failure, Chronic Kidney Disease, and Other Complex Illnesses
  • Accurately Document Patient Encounters to Support Medicare Advantage, Risk Adjustment (MRA), and RAF Initiatives
  • Monitor and Improve HEDIS Measures, Quality Metrics, Preventive Care Compliance, and Care Gap Closure
  • Develop Individualized Treatment Plans Focused on Improving Patient Outcomes and Reducing Hospitalizations
  • Coordinate Care with Specialists, Hospitals, Home Health Agencies, and Other Healthcare Providers
  • Educate Patients and Families Regarding Diagnoses, Treatment Plans, Medications, Lifestyle Modifications, and Preventive Health Strategies
  • Review Laboratory and Diagnostic Results and Provide Appropriate Follow-Up Care
  • Participate in Quality Improvement Initiatives and Value-Based Care Programs
  • Collaborate with Advanced Practice Providers, Nurses, Care Coordinators, and Administrative Staff to Deliver Exceptional Patient Care
