Reporting to the Director of Pharmacy with matrix collaboration to ambulatory medical directors, the Collaborative Drug Therapy Management Pharmacist partners closely with friendly nurses and a diverse interprofessional team—physicians, APPs, case management, social work, respiratory therapy, and quality—while leading protocol-driven CDTM services in chronic disease management (diabetes, hypertension, heart failure, COPD/asthma), transitions-of-care medication reconciliation, MTM, anticoagulation, antimicrobial stewardship, population health initiatives, prior-authorization optimization, and evidence-based order set development; as a CDTM Ambulatory Pharmacist, expect optimized workflows, analytics dashboards, mentorship opportunities, precepting of learners, and leadership that listens through regular feedback loops and frontline-driven improvement.
