Job description

Schedule

  • Mon–Fri, 8:00am–4:30pm

  • 40 hrs/week

Role Overview


  • Manage patients with chronic comorbidities, primarily CKD and ESRD/ESKD

  • Perform annual wellness exams in‑home and virtually

  • Coordinate care with PCPs, specialists, and ancillary services

  • Prescribe medications, order tests, and collaborate with physicians

Requirements

  • 2+ years of direct patient care

  • FNP, AGNP, or ACAGNP

  • Board‑certified (ANCC or AANP)

  • Active DEA

  • Active Georgia NP license

  • Nephrology experience preferred
