We are a Medicare-certified home health agency committed to providing compassionate and personalized care in the comfort of our patients’ homes. As we continue to grow, we are seeking a skilled and motivated Physical Therapist to join our team and help improve patient outcomes across San Bernardino, Riverside, and Orange County.

Responsibilities:

  • Perform in-home evaluations and develop individualized care plans

  • Provide physical therapy services in accordance with each patient's plan of care

  • Educate patients and caregivers on exercises, mobility strategies, and safety

  • Collaborate with physicians, nurses, and other clinicians to ensure coordinated care

  • Complete timely and accurate clinical documentation

Qualifications:

  • Current and valid California Physical Therapist license

  • Minimum 1 year of experience (home health preferred, but not required)

  • Reliable transportation and a valid driver’s license

  • Strong communication, clinical judgment, and time management skills
