About Us:

We are a Medicare-certified home health agency dedicated to delivering compassionate and personalized care in the comfort of our patients’ homes. We are currently seeking a dependable and motivated Physical Therapist Assistant (PTA) to support patient rehabilitation and improve mobility and quality of life across San Bernardino, Riverside, and Orange County.

Responsibilities:

  • Provide in-home physical therapy treatments as directed by a supervising Physical Therapist

  • Assist with implementing care plans and documenting patient progress

  • Educate patients and caregivers on home exercise programs and mobility support

  • Communicate changes in condition or response to therapy with the PT and care team

  • Complete accurate and timely clinical documentation

Qualifications:

  • Current and valid California PTA license

  • Minimum 1 year of experience (home health preferred, not required)

  • Reliable transportation and valid driver’s license

  • Strong interpersonal, clinical, and organizational skills
