We are seeking a Part\-Time Nurse Practitioner (NP)<\/b> to provide inpatient medical care in a Skilled Nursing Facility. This is an excellent opportunity for a provider who enjoys building relationships with patients, collaborating with interdisciplinary teams, and maintaining a flexible work\-life balance.

  • Manage and round on patients in a Skilled Nursing Facility

    • Perform daily inpatient medical rounds in a Skilled Nursing Facility.
      <\/li>\n
    • Evaluate, diagnose, and manage acute and chronic medical conditions.
      <\/li>\n
    • Develop and implement individualized treatment plans.
      <\/li>\n
    • Coordinate care with physicians, nursing staff, therapists, specialists, and family members.
      <\/li>\n
    • Complete timely and accurate documentation within the EMR.
      <\/li>\n
    • Participate in quality improvement initiatives and ensure compliance with facility and regulatory standards.
      <\/li>\n
    • Provide compassionate, patient\-focused care that promotes positive clinical outcomes and reduced hospital readmissions.
      <\/li>\n <\/ul>\n Qualifications
      <\/h3>\n
        \n
      • Current Nurse Practitioner license in the state of practice.
        <\/li>\n
      • National Board Certification (AANP or ANCC).
        <\/li>\n
      • DEA license (or ability to obtain).
        <\/li>\n
      • Experience in Skilled Nursing, Geriatrics, Internal Medicine, Family Medicine, Hospital Medicine, or Post\-Acute Care preferred.
        <\/li>\n
      • Excellent communication and clinical decision\-making skills.
        <\/li>\n
      • Ability to work independently while collaborating with an interdisciplinary care team.
