Job description

Our company is an established home healthcare company that has been servicing patients in Illinois seeking a high-energy, self-initiating, and caring RN who can work as a field nurse Chicago, IL.  Areas of coverage needed  Your organizational skills and strong understanding of home health, geriatric care, and previous experience.  Area of coverage: 60607 60608 60609 60610 60612 60614 60622 60623 60624 60644 60647

  Skill set:  IV teaching, PICC line care, foley care, feeding tube care, chest tube care, ostomy care, wound care and wound vacs.  

Responsibilities and Qualifications:

As a Registered Nurse RN you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

Additional responsibilities of our Registered Nurse include:

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients. 
  • Confer with the physician and patient in developing a plan of care based on patient's medical condition and assessment findings at the time of the Start of Care and with every re-certification
  • Observe and assess patient's overall health and community status; train/educate patient /family on disease condition and treatment plan; render hands on care as needed and do management and evaluation when appropriate.
  • Coordinate with other disciplines involved in case to ensure that quality services are provided and there is continuity of care among providers
  • Update physician on patient status routinely and as needed when patient has a significant change in condition
  • Work with families and get them involved in the plan of care in order to achieve maximum resolution to patient needs/problems.
  • Create plan of care for home health aides and supervise them routinely to ensure that they are following the plan of care.
  • Perform discharge planning and provide patient with the necessary information they require in order for them to be able to continue to manage on their own after home health services are discontinued.
  • Get involved with QI projects and offer input to help improve delivery of service • Adhere to the Nursing Standards of Practice.

Required Skills

  • Associates of Science degree in Nursing
  • Valid RN license in state of employment
  • Valid driver's license
  • Able and willing to travel
  • Ability to successfully complete required background check and drug screen
  • Excellent writing and charting skills, experience with electronic medical record preferred
